How to Register at our Practice

New patients are very welcome.  To register with the Practice please report to reception where you will be given a form to complete your registration. You can also download this form by clicking on the following link, print and complete; bring the form with you when registering.

NHS GP Registration form (GMS1)

We take patients for both doctors in the area shown on our Catchment page.

You will require two forms of identification:

  • a passport or Full driving Licence (both parts)
  • a utility bill with your name and address on it

We will also need to confirm that you live within the Practice area.  When making an appointment you may request a consultation with the Doctor of your choice.

Temporary Residents

We are always happy to see any relatives or friends staying with you if they need a Doctor or Nurse.  They will need to fill out a short form for the information to be returned to their own Doctor.  Please arrive 10 minutes before your appointment to allow time for completion of the necessary forms.


Here at Deneside Medical Centre  we always aim to treat patients in a comfortable and safe environment with dignity and respect,  to ensure that you receive the best possible care. All patients are entitled to have a chaperone present for any consultation if they wish, this could be for a discussion, examination or procedure if they feel this is necessary The chaperone may be a family member or friend or it can be a member of the clinical staff or administration staff.

If you would like a chaperone present then please ask a member of the team when booking an appointment or ask the clinician prior to commencing the appointment. There may be times when the Clinician feels a chaperone is required and your consent will be obtained in order for this request to happen. All staff have been updated on the role of a chaperone and are able to carry out this role with dignity.
